NTSB Factual Report

On July 25, 2018, about 0930 Pacific daylight time, a Piper PA-32-300 airplane, N4747H, overran the runway and collided with terrain near Avalon, California. The private pilot sustained serious injuries and the two passengers sustained minor injuries. The airplane was registered to and operated by the pilot under the provisions of Title 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed at the airport about the time of the accident, and no flight plan was filed. The flight departed Fullerton Municipal Airport (FUL), Fullerton, California, about 0904.

According to the airport manager, the wind about the time of the accident was 330° at 4 knots. He reported that while the airplane was in the downwind for runway 22, he advised the pilot of the wind and recommended right traffic pattern. He stated that the pilot's first landing attempt was high and the airplane didn't land. After a go-around, and on the second landing, the airplane was "still to high and came in too fast" but was able to touchdown about midfield. As the airplane neared the end of the runway, he heard the engine "rev up" and observed the airplane depart the runway in a "very nose high" pitch attitude. The airplane subsequently collided with terrain about 500ft beyond the departure runway.

At the time of this report, the pilot was not available to be interviewed.
